All forms below are in Adobe (.pdf) format. The individual forms should only be filled out and submitted to register if you already have a Participant Package on file with the Academy. If you are new to the Academy (AAES or Camp Koda) and are signing up for a program, service, etc. for the first time, you need to fill out and submit the Participant "Package" also which contains a Liability Waiver, Health History and Program Policies documents.
